• Outreach patients to schedule necessary appointments.
• Assist in overcoming barriers to care such as transportation or financial challenges.
• Retrieve diagnostic and referral results to ensure comprehensive documentation and continuity of care.
• Monitor, update, and follow up on patient referrals using external referral management systems, ensuring patients are scheduled with outside specialists, services, or community partners.
• Address patient navigation needs flagged within internal electronic work queues, providing high‑level support and guidance throughout the care continuum.
• Identify and reduce barriers to care; provide direct support or connect patients to community‑based resources.

• High School Diploma or GED Equivalent.
• Associate’s or Bachelor’s Degree (preferred). Graduate of an accredited school of Practical Nursing preferred.

• Experience in healthcare, health center or clinical case management (preferred).

• MA, LPN, RN (preferred).
• BLS certification within the first 6 months (employer provided).
