- Outreach patients to schedule necessary appointments.
- Assist in overcoming barriers to care such as transportation or financial challenges.
- Retrieve diagnostic and referral results to ensure comprehensive documentation and continuity of care.
- Monitor, update, and follow up on patient referrals using external referral management systems, ensuring patients are scheduled with outside specialists, services, or community partners.
- Address patient navigation needs flagged within internal electronic work queues, providing high‑level support and guidance throughout the care continuum.
- Identify and reduce barriers to care; provide direct support or connect patients to community‑based resources.
- High School Diploma or GED Equivalent.
- Associate’s or Bachelor’s Degree (preferred). Graduate of an accredited school of Practical Nursing preferred.
- Experience in healthcare, health center or clinical case management (preferred).
- MA, LPN, RN (preferred).
- BLS certification within the first 6 months (employer provided).
