Job Summary: The Physical Therapist provides comprehensive physical therapy services to the patient by assessing, planning, implementing, and documenting patient care and treatment programs.

RESPONSIBILITIES:

• Provides comprehensive physical therapy services to the patient by assessing, planning, implementing, and documenting patient care and treatment programs.
• Participates in varied Rehabilitation Services department activities designed to enhance patient care and programs.
• A Physical Therapist II demonstrates advanced evaluation and treatment skills and is involved in program development, staff education, student supervision.
• Also demonstrates emerging leadership skills and mentors department staff.

REQUIREMENTS:

• A minimum of two years of clinical Therapist experience meeting requirements of Level I responsibilities as determined by interview and assessment.
• Bachelors, Masters, or Doctorate degree from an accredited Physical Therapy program is required.
• WI state license as a Physical Therapist is required.
• American Heart Association (AHA) Basic Life Support for the Healthcare Provider (BLS) or an AHA approved equivalent is required within 3 months of hire.
