In Horizon’s Hospice Inpatient Units, you will play an important role in supporting and educating patients and families as they embark on the end‑of‑life journey. You will draw on your clinical knowledge, experience, and compassion to provide care that aligns with our hospice philosophy.
Responsibilities- Perform referral evaluations / admission assessments of patients and families seeking hospice services.
- Conduct physical, psychosocial and spiritual assessments, utilizing non‑verbal cues and probing for comprehensive information.
- Communicate patient and family expectations, goals and learning needs to the health care team via appropriate documentation.
- Maintain documentation that is clear, legible, concise, accurate, timely, and complies with department standards.
- Provide case‑management functions as part of the Integrated Delivery Group to an assigned caseload.
- Approach patients and families with care, concern and compassion while providing services at quality levels consistent with professional practice standards.

- Degree from an accredited School of Nursing (BSN preferred).
- Current WI RN license.
- 1–2 years of hospice, oncology or related experience.
- Experience in the care of the terminally ill; knowledge of hospice care.
- Hospice certification required within 18 months of employment.
- Current/valid WI driver’s license with reliable/insured vehicle.
Full‑time – 40 hours/week – 3 pm to 11:30 pm, with alternating weekend shifts.
