RESPONSIBILITIES- Develop trusting relationships with patients, families, and your community.
- Deliver nursing care and take full responsibility for the delivery of the Plan of for your home health patients, making necessary changes to maintain quality of care.
- Make the initial nursing evaluation and create an appropriate and patient‑specific plan of care.
- Identify the patient’s/family's physical, psychosocial, and environmental needs and tailor each care plan appropriately.
- Complete recertifications or resume care for eligible patients.
- Initiate and coordinate the plan of care while maintaining accurate and relevant clinical notes regarding the patient’s condition.
- Document problems, appropriate goals, interventions, and patient/family response to skilled nursing care.
- Collaborate with the patient/family, attending physician, and other members of the IDG in providing patient and family care. /ul>LICENSE & EXPERIENCE REQUIREMENTS
- One year of clinical experience preferred.
- Current Wisconsin RN licensure.
- Current’s license, valid vehicle insurance, and access to a dependable vehicle.

Location:
Sheboygan and Ozaukee Counties.
Schedule:
Full‑Time, 1st shift Monday‑Friday. Minimal on‑call required.
Compensation:
Negotiable based on experience.
Sign‑On Bonus:
Up to $7,500 available, depending on location and experience.
