We are seeking a Home Health RN Case Manager to provide care in Sheboygan and Ozaukee Counties

• Develop trusting relationships with patients, families, and your community.
• Deliver nursing care and take full responsibility for the delivery of the Plan of for your home health patients, making necessary changes to maintain quality of care.
• Make the initial nursing evaluation and create an appropriate and patient‑specific plan of care.
• Identify the patient’s/family's physical, psychosocial, and environmental needs and tailor each care plan appropriately.
• Complete recertifications or resume care for eligible patients.
• Initiate and coordinate the plan of care while maintaining accurate and relevant clinical notes regarding the patient’s condition.
• Document problems, appropriate goals, interventions, and patient/family response to skilled nursing care.
• Collaborate with the patient/family, attending physician, and other members of the IDG in providing patient and family care.
/ul>LICENSE & EXPERIENCE REQUIREMENTS
• One year of clinical experience preferred.
• Current Wisconsin RN licensure.
• Current’s license, valid vehicle insurance, and access to a dependable vehicle.
LOCATION & SCHEDULE

Location:

Sheboygan and Ozaukee Counties.

Schedule:

Full‑Time, 1st shift Monday‑Friday. Minimal on‑call required.

COMPENSATION & SIGN‑ON BONUS

Compensation:
Negotiable based on experience.

Sign‑On Bonus:
Up to $7,500 available, depending on location and experience.
