Role: RN Case Manager—discharge planning, care coordination, continuum of care, patient focus and care plans, and resource utilization.

MINIMUM REQUIRED QUALIFICATIONS

• CA RN license
• BLS certification
• 2‑3 years recent acute hospital case management experience
• Discharge planning experience

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – M‑F 8:00 am‑4:30 pm with occasional weekends

START DATE – 1/12/2026

PAY RATE – $1,800.00 – $2,700.00 per month
