JOB SUMMARY

Provides high quality primary care to individuals, families, and groups of Community Health Center clients according to established professional and medical standards. Works collaboratively with other health care team members to meet both acute care, and health maintenance needs of clients. Maintains hospital privileges as necessary according to local institution's standards and requirements. Maintains professional licensure as appropriate. Fully established practice standards for productivity and appropriate utilization of care.

Position is designed to support evening and or weekend hours.

ROLE AND RESPONSIBILITIES Clinical

Provides ongoing high quality primary care to clients. Performs clinical procedures competently. Cares for acutely ill clients in inpatient or emergency room setting as necessary. Works collaboratively with members of the interdisciplinary team providing medical support and consultation as necessary. Maintains and documents the problem oriented medical record accurately after each client visit and monitors health maintenance checks of patient panel.

Participates in other health care activities as agreed, e.g. school based clinic consultant, SNF care.

Administrative

Maintains board certification/licensure. Attends continuing education programs. Maintains involvement in professional organizations as necessary. Meets hospital medical staff requirements as necessary. Collaborate with office manager and clerical/clinical staff to promote the smooth functioning of the medical component meeting client’s health care needs. Participates with Community Health Center management in developing and achieving optimal clinical performance and organizational goals for productivity and utilization.

QUALITFICATIONS

AND PREFERRED SKILLS

APRN:
Graduate of an accredited Master of Science in Nursing program with a specialty as a family nurse practitioner. Maintains board certification as a family nurse practitioner. Maintains appropriate board certification, licensure, and professional organizations.

PA:
Graduate of an accredited Master program in the study of physician assistant. Maintains appropriate board certification, licensure, and professional organizations.

CHC requires as a condition of employment current American Red Cross CPR for the Professional Rescuer and AED (CPR/FPR/AED) certification. The only acceptable alternative is current American Heart Association BLS/AED for Healthcare Providers certification.

Communicates effectively with clients and staff. Documents interaction clearly and concisely in the medical record. Requires effective communication skills at all levels of interaction. Required to develop bilingual capacity as dictated by needs of the site.

PHYSICAL REQUIREMENTS /WORK ENVIRONMENT

Moderate physical activity level, i.e. walking, standing. Potential blood borne pathogen/chemical hazard exposure.

WORK SCHEDULE DEMANDS

Hours as agreed. Clinical schedule arranged to include office hours, hospital rounds, etc. Scheduling options will include 2 evenings and/or weekend coverage (currently full day Saturday and ½ day Sunday).
