Primary Care Support RN – Meriden – 8 hours
8 hours a week, Thursdays 8am – 5pm
Responsibilities- Provide high quality, comprehensive primary nursing care to individuals and families throughout the lifespan.
- Work at the top of licensure to enhance safety, effectiveness, and efficiency of care through professional responsibilities and delegation of technical and clerical functions to medical assistants.
- Provide general and focused health education regarding preventive, chronic, and episodic health care issues.
- Collaborate with primary care providers to coordinate care, support transitions between levels of care, counsel, educate, and support patients in improving health status.
- Apply principles of advanced access, planned care, chronic care model, and rapid cycle performance improvement using PDSA at CHCI.
- Support primary care under the planned care model; identify needs for preventive care, evidence‑based chronic disease care, and care coordination.
- Assist providers with exams, treatments, and procedures as needed; follow up on emergency room visits and hospital discharges.
- Administer medications, including vaccines and immunizations, orally, subcutaneously, intramuscularly, intradermally or via nebulizer under standing orders; monitor patient response and provide education.
- Perform wide range nursing activities as needed (wound care, catheterization, etc.); accurately document all patient care in the medical chart promptly.
- Maintain strict infection control standards, universal precautions, OSHA, CLIA; monitor and ensure follow‑up of abnormal Pap Smear results, STDs, lead screens and other tests at CHCI.
- Oversee supplies and equipment; order supplies as necessary; maintain compliance with agency mandatory annual training (Patient Safety, Abuse, Age‑Based Competencies, EOC, Abuse trainings).
- Expert use of electronic health record to ensure timely, safe, confidential storage and retrieval of patient information.
- Bachelor of Science in Nursing (BSN) OR Associates of Science in Nursing (ASN).
- Licensed Registered Nurse (RN) with experience in primary care or community health setting preferred.
- Strong clinical knowledge of preventive, chronic disease, and episodic health care issues.
- Excellent communication skills and ability to educate patients and families.
- Proficiency in electronic health record (EHR); experience with CHCI protocols preferred.
- Ability to work independently and collaboratively on a part‑time schedule (8 hours per week).
