The Home Health LPN Case Manager provides skilled nursing care and coordinates care for patients in their homes, ensuring safe and individualized treatment plans. The role involves managing a caseload, conducting assessments, and collaborating with the interdisciplinary team to support patients and families. The position is based in the Boise metropolitan area and offers opportunities for compassionate, patient-centered care.
Key Responsibilities:Provide skilled nursing care to patients in their homes per physician orders and individualized plans
Serve as LPN case manager for an assigned home health patient caseload
Perform patient assessments and monitor changes in physical, functional, and psychosocial status
Provide treatments, medication administration, wound care, and other nursing services within LPN scope
Monitor patient progress and communicate significant changes to supervising RN, physician, or appropriate professional
Assist with developing and maintaining individualized patient care plans
Coordinate patient care with registered nurses, physicians, therapists, aides, social workers, and other team members
Educate patients and caregivers about medications, treatments, disease management, safety, and home care practices
Reinforce teaching provided by RN and other interdisciplinary team members
Provide compassionate support to patients and families while promoting independence and quality of life
Complete accurate and timely clinical documentation per agency, state, and federal requirements
Manage a field-based schedule efficiently while traveling between patient homes in the Boise area
Current and unrestricted Licensed Practical Nurse (LPN/LVN) license in Idaho or eligibility for Idaho licensure
Minimum 1 year of home health experience
Graduation from an accredited Practical Nursing or Vocational Nursing program
Current BLS/CPR certification for healthcare providers
Valid driver's license
Reliable transportation for travel to patient homes
Ability to meet applicable automobile insurance requirements
Strong clinical, organizational, communication, and documentation skills
Skills:
Previous experience as a Home Health LPN Case Manager or Home Health LPN
Experience providing skilled nursing care in patients' homes
Familiarity with Medicare home health services and documentation requirements
Experience with wound care, medication administration, patient education, and chronic disease management
Strong understanding of care coordination and interdisciplinary healthcare
Experience working with electronic medical records
Ability to work independently while maintaining strong communication with the clinical team
Compassionate, patient-centered approach to nursing care
Strong time-management and organizational abilities
