Registered Nurse Case Manager provides skilled nursing care and coordinates home health services for patients in Meridian, Idaho. The role ensures safe, independent, and high-quality care by assessing needs, developing individualized plans, and collaborating with an interdisciplinary team. This position supports patients in achieving health goals within their homes.
Key Responsibilities:Provide skilled nursing care to patients in their homes, promoting safety, independence, and quality of life.
Serve as primary RN Case Manager and coordinate overall care plans for assigned patient caseload.
Perform comprehensive nursing assessments and evaluate patients' physical, psychological, and social needs.
Develop, implement, and regularly update individualized plans of care based on patient needs and physician orders.
Coordinate care with physicians, therapists, social workers, aides, caregivers, and other interdisciplinary team members.
Complete accurate and timely clinical documentation in accordance with agency, state, and federal requirements.
Perform and document OASIS assessments as required for home health patients.
Monitor patient progress and identify changes requiring intervention or physician notification.
Educate patients and family members regarding medications, disease processes, treatment plans, symptom management, and home care.
Review medications and identify potential medication-related concerns or discrepancies.
Communicate effectively with patients, families, physicians, and other healthcare professionals.
Promote continuity of care throughout the patient's home health episode.
Current and unrestricted Registered Nurse license in Idaho or eligibility for licensure.
Minimum 1 year of Home Health RN experience.
Current BLS/CPR certification for healthcare providers.
Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited program.
Valid driver's license and reliable transportation for traveling to patient homes.
Ability to maintain appropriate automobile insurance as required by law.
Strong clinical assessment, communication, organization, and critical-thinking skills.
Home health, Medicare, OASIS, and care coordination experience strongly valued.
Skills:
Previous experience as a Home Health RN Case Manager.
Experience completing OASIS assessments and documentation.
Familiarity with Medicare home health regulations and requirements.
Experience coordinating interdisciplinary patient care.
Familiarity with home health electronic medical records and documentation systems.
Ability to work independently while remaining connected with the clinical team.
Compassionate, patient-focused approach to nursing care.
You have at least one year home health RN experience. You hold a current, unrestricted Idaho RN license. You are skilled in OASIS assessments and care coordination
