St. Luke’s Health System seeks a Home Health Case Manager to collaborate with physicians and the care team to plan and manage individualized in-home care for a designated caseload. This direct care role requires travel within Boise, surrounding regions from admission to discharge.
The RN must be licensed in Idaho with BLS, and prior Home Health or Acute Care experience is preferred. Full-time hours include a 8:30 AM–5:00 PM schedule with on-call and weekend coverage as needed.
