- Adult Care Coordination Program

For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. Catholic Charities provides individuals with serious mental illness, complex medical needs and substance abuse issues with comprehensive care coordination and case management services.

Our Care Coordination team works with the New York State Health Homes Program to help adults meet their health and wellness goals, and live self-directed lives within their community.

RESPONSIBILITIES

The Health Home Team Supervisor oversees a team of direct care staff charged with coordinating all aspects of care for assigned clients. Clients present with mental health needs, complex medical conditions, co-occurring substance use disorders, and a wide array of social service needs. In addition to overseeing client-centered care, the Team Supervisor has additional goals of reducing avoidable health care costs, specifically preventable hospital admissions/readmissions and avoidable emergency room visits, and improving client’s health outcomes by addressing primary medical, specialist and behavioral health care through contractual arrangements with appropriate service providers.

The Team Supervisor is a driving force behind comprehensive, integrated services, ensuring direct care staff conduct a comprehensive assessment of clients and facilitate access to the full range of medical, behavioral health, substance use, social and psychosocial services in the community, in a timely, efficient, and effective manner.

Clients are provided care coordination services in their home and in the community. The program applies a person-centered and recovery-oriented approach to care, empowering the individual to be active participants in their healthcare journey. The Team Supervisor also maintains health home networks with primary medical and specialty practitioners and mental health providers, substance abuse service providers, community-based organizations, managed care plans, emergency rooms, hospitals, and residential/rehabilitation settings to ensure coordinated, and safe transitions of in care for clients who require transfer to/from various levels of care.

The Team Supervisor monitors daily activities of staff, ensuring the scope, frequency and duration of client encounters are adequate to meet client needs, and substantiate monthly billing of core services. This includes oversight of progress notes documentation, assessments, Care Plans, and billing tools through regular chart reviews and staff supervisions. The Team Supervisor also ensures implementation of a continuous quality improvement plan for the team and conducts workshops and trainings as needed.

HH Team Supervisor is required to utilize technology and various web-based platforms for documenting progress notes and daily work activities. Ability to use equipment such as iPhones, tablets, Surface Pros and easily navigate various technology platforms and reporting systems is a requirement.

QUALIFICATIONS
  • Master’s Degree in Social Work, Nursing, Public Health or other related social service or health profession (NYS RN, NP, LMSW/LCSW CASAC, License or a Licensed Psychologist preferred)
  • The position requires a combination of skills in the areas of crisis intervention, time management, psychosocial rehabilitation skills.
  • Must have excellent communication skills.
  • Cross-cultural competency, outreach, interviewing, listening, advocating, linking, negotiating, engagement, monitoring and clinical assessment skills are essential.
  • Excellent computer skills are necessary.
  • Knowledge of the community medical resources and their financial requirements.
  • Good oral and written communication skills.
  • Fluency in second language preferred
