- Graduate of an accredited school of nursing.
- Associates degree in nursing.
- Registered as RN with New Hampshire’s Board of Nursing.
- A minimum of one (1) year recent acute care experience.
- Ability to exercise initiative and independent judgment.
- Excellent verbal and written communication skills and competent in problem solving abilities.
- Admission to HHHC's homecare division.
- Completes an initial home health assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
- Utilizes the nursing process in planning, providing, and evaluating skilled needs.
- Completes a comprehensive patient assessment that is reflected in accurate OASIS scoring.
- Develops a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.
- Bachelor’s degree in nursing.
- Community Health/Education experience.
- Certifications, which may include: IV;
Chemotherapy;
Ostomy. - Familiarity with OASIS and what drives reimbursement.
- Allscripts’ EMR software proficiency.
- Supervisory experience over home health aides.
- CPR Certification.
- Satisfactory criminal record.
- Satisfactory motor vehicle record.
