Responsibilities
  • Completes an initial home health assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Utilizes the nursing process in planning, providing and evaluating skilled needs.
  • Completes a comprehensive patient assessment that is reflected in accurate OASIS scoring.
  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.
  • Case management of your patients! You are key to helping them heal, getting back to feeling strong and healthy!
Qualifications
  • Graduate of an accredited school of nursing.
  • Associate degree in nursing.
  • Registered as RN with New Hampshire’s Board of Nursing.
  • One years’ recent acute care experience. Preferably in med/surg or acute rehab setting.
  • Ability to exercise initiative and independent judgment.
Preferred Qualifications
  • Bachelor’s degree in nursing.
  • Community Health/Education experience.
  • Certifications:

    IV, Chemotherapy, Ostomy, as examples.
  • Familiarity with OASIS and what drives reimbursement.
  • EMR software proficiency.
Required
  • Registered as a RN with New Hampshire’s Board of Nursing.
  • CPR Certification.
  • Satisfactory criminal record.
  • Satisfactory motor vehicle record.
