The primary purpose of this position is to assist the Admissions Director in developing census and quality mix by providing prospective residents and/or responsible parties with appropriate information and assistance to the nursing center, in accordance with current federal, state, and local standards, guidelines and regulations, and Solaris Health Care’s established policies and procedures.
General Responsibilities- Complete all assigned tasks in a timely manner as instructed by supervisor.
- Adhere to work schedules in completing and performing assigned tasks.
- Cooperate with department personnel, as well as other facility personnel to ensure that services can be adequately maintained to meet the needs of residents.
- Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout Solaris Health Care.
- Treat residents, residents’ family members, visitors and fellow employees with courtesy, respect and dignity.
- Meet with Supervisor on a regular basis to assist in identifying and correcting problem areas and/or improving services.
- Notify facility when late arrival to or absence from work according to established center policy.
- Adhere to established employee policies.
- Adhere to established dress code.
- Meet acceptable center attendance requirements.
- Ensure that all care is provided in privacy.
- Alerts residents before entering resident rooms.
- Report all grievances and complaints made by residents to supervisor.
- Report all allegations of resident abuse and/or misappropriation of resident property.
- Handle walk‑in or telephone inquiries and document on the inquiry logs.
- Follow up on inquiries by contacting case managers via telephone or through the electronic referral systems.
- Conduct tours of the center, document clinical and financial information on the inquiry form for follow‑up. All tours must be provided a brochure and the complete contact information must be secured for appropriate follow‑up.
- Participate in community outreach events held at the facility by greeting guests, setting up for the events, offering tours and any other duties as assigned.
- Use formal waiting list policy and keep future reservations for residents.
- Review a list of unoccupied beds and make preadmission reservations according to type of case and accommodation required.
- Ensure that a model room is maintained in the nursing center.
- Maintain necessary knowledge of served market areas, resident origin, and major referral sources.
- Gather demographic or clinical information from either the social worker and/or floor nurse at hospital or the family of the admitting patient prior to admission.
- Enter new patient information into Matrixcare. Scan/uploads all required documents into Matrixcare in a timely manner.
- Verify primary and/or secondary insurances, check FDLE, etc.
- Assume responsibility for timely and accurate completion of admission contract. Solaris guideline is within 48 hours.
- Explain differences in rates and charges to residents desiring changes of accommodations, and arrange for the change; explain services and nursing home policy regarding payment of bills; provide copies for families regarding limited liability treatment policy and legal surrogate; give families…
