MDS Coordinator Role

The MDS Coordinator will assist the Director of Nursing with ensuring that all documentation in the center meets Federal, State, and Certification guidelines. The MDS Coordinator will coordinate the entire RAI process assuring the accuracy, timeliness, and completeness of the MDS, CAAs, and Interdisciplinary Care Plan.

Qualifications
  • Health – Ability to meet performance requirements.
  • Education,

    Experience and Skills:
    • Graduate from an approved school of nursing, College or University.
    • Must have a current, unencumbered license to practice nursing as a Registered Nurse/Licensed Practical Nurse in Florida.
    • Participates in continuing education so that he/she has knowledge of current trends and developments in nursing and maintains current license.
    • Minimum of two years of RAI and Care Plan experience, preferably in a Skilled Nursing Facility (SNF).
    • Must have knowledge of MDS 3.0 including Assessments, CAAs and Care Plans.
    • Conduct each Assessment and Care Plan with the appropriate participation of health professionals.
    • Ability to organize work procedures, assume responsibility, and tactfully interact with other disciplines.
    • Must be able to communicate well with any member of the Interdisciplinary Team.
    • Collaborate with the IDT to provide guidance on appropriate and precise charting for the medical record.
Performance Requirements
  • Must be able to sit, stand, bend, twist, lift and move intermittently throughout the day.
  • Must be able to lift, push, pull and otherwise move a minimum of 50 pounds.
  • Able to see and hear adequately in order to respond and interact with other staff and families.
  • Able to speak in clear, concise voice in order to communicate with patients, families, and other staff and disciplines.
  • Relate to and work with the elderly, disabled, emotionally upset and hostile Residents.
  • From time to time, you may be required to assume the duties of a floor nurse.
Specific Duties & Responsibilities
  • Assign responsibility for completing sections of the MDS to staff who have clinical knowledge about the patient.
  • Assure that non-licensed staff who provide direct care to the patients are allowed to contribute information for the assessment process.
  • Conducts, completes or coordinates other disciplines in completion of the RAI and certifies the completion of the Assessment.
  • Sign and certify the CAA Summary Form.
  • Attend care planning and enter the care plan decision information.
  • Assist with month-end duties for closeout.
