Provide appropriate information and/or undertake a proportionate assessment and promote a culture of independence and positive outcomes when signposting or commissioning services.
Responsibilities- Be part of a busy Duty Team and carry out assessments in line with the Social Service and Well-being Act 2014.
- Cover the duty rota on designated days and provide absent coverage if required.
- Engage in a structured conversation with those making an initial enquiry of the service and evaluate any information given.
- Give information, advice and/or sign‑post to other services.
- Ensure that information about advocacy services is provided.
- Accurately and fully record referral information in accordance with departmental procedures.
- Identify any need for advocacy to support the service user to participate in the IAA service proportionate assessment.
- Identify safeguarding concerns and take appropriate actions.
- Carry out community care assessments to ensure service users are receiving the required services to meet their needs, including packages of care, day services, respite and linking with 3rd sector and signposting to other agencies where appropriate.
- Respond to all 126 enquiries and undertake a visit if deemed necessary.
- Agree a range of solutions to meet identified outcomes with adults and carers by providing short‑term respite for service users in a residential setting.
- Approach and undertake Mental Capacity assessments where appropriate to fulfil the requirements of the MCA code of practice.
- Undertake reviews in accordance with departmental policies.
- Evidence of all essential qualifications (details to be provided in application).
- Ability to speak Welsh is desirable.
