Hospice LPN (Licensed Practical/Vocational Nurse)

Qualifications
  • Current LPN/LVN license in the state of Arizona (or compact license)
    ● Current CPR certification
  • Excellent communication and interpersonal skills
  • Professional demeanor and appearance
Responsibilities
  • Follow established policies, procedures, and the IDG care plan for each patient
    ● Provide nursing services, treatments, and preventive care as assigned
    ● Observe and report changes in patient condition to the RN and physician
    ● Educate and support patients and families in home care and comfort techniques
    ● Collaborate with the RN to evaluate patient care effectiveness
  • Maintain accurate documentation and participate in chart audits
  • Attend staff meetings, IDG conferences, and in‑service training
  • Participate in on‑call rotations (nights, weekends, holidays as needed)
    ● Prepare equipment and materials, ensure aseptic techniques, and assist in specialized procedures
