The Licensed Practical Nurse is responsible for utilizing the nursing process to deliver high-quality, continuous nursing care to patients in their homes.

- Complete and scan required record-keeping forms upon patient admission, transfer, and discharge.
- Chart nurses' notes clearly, reflecting the care provided and patient responses.
- Report discrepancies concerning physician’s orders, diet changes, or charting errors to the Director of Nursing.
- Document unauthorized discharges appropriately in the medical records as per procedures.
- Current, unencumbered LPN/LVN license or Compact State license.
- Minimum of 2 years LPN/LVN experience.
- Ability to educate patients and families on disease processes, with good medication knowledge and wound care experience.
