Casa de la Luz East Valley, a part of LHC Group in the Optum family, is seeking a Registered Nurse Case Manager to deliver hospice care and develop the Plan of Care for patients and families. You will evaluate patient needs, coordinate with the IDG, and implement treatment strategies to promote physical, psychosocial, emotional, and spiritual well-being.
Current RN licensure and a valid driver's license are required; CPR certification is also needed, with certification achievable within 90 days
