Social Service Coordinator

Responsibilities

• Complete social service assessments and progress notes for facility residents.
• Coordinate discharge plans with home health agencies and medical equipment vendors.
• Assist residents and family members with Medicaid applications and paperwork.
• Lead care plan conferences alongside nursing leadership and therapy staff.
• Address daily resident non-clinical needs and coordinate with floor staff on care updates.

Compensation, Schedule & Benefits

• Pay Rate:
  Starting at $34/hr based on experience
• Schedule:
  
  Mon–Fri (Day)

Qualifications & Requirements

• Active Texas Social Work License (LSW, LMSW, LBSW, or LCSW)
• Long-term care or skilled nursing facility experience preferred
• Must pass a standard criminal background check
• Must pass a drug screening
• Reliable transportation for consistent attendance
