LPN Med Care Manager

The Licensed Practical Nurse (LPN) Medication Nurse provides licensed nursing support in senior living settings, focusing on safe medication administration, health monitoring, and accurate clinical documentation.

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration
• Document medication administration in MAR/TAR in real time and accurately
• Monitor residents’ symptoms, report adverse reactions, and support care plan updates
• Observe physical, emotional, or cognitive changes and communicate findings promptly
• Maintain medication carts, conduct narcotic counts, and ensure regulatory compliance
• Conduct Service & Health Updates (SHUs) and transcribe physician orders

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• Preferred experience in senior care or medication administration
