Home Health – Physical Therapist (Days)
Location: Methuen, MA (servicing surrounding territories incl. Southern NH)
Employment Type: W2 Contract
Start: ASAP | Length: 13 weeks
Schedule: Days | Hours: 40/week

Requirements

• Active MA & NH PT licenses (dual required)
• BLS
• 2+ years PT experience
• True Home Health experience required
• Homecare Homebase (HCHB) + OASIS required
  • OASIS-E (post Jan 2023) required
• No more than 1-year work gap

Assignment Details

• ~20–25 visits/week | 5–7 points/day
• Productivity avg: 26–28 points
• Same-day documentation required (completed in the home)
• Float flexibility required
• No on-call | No OT
• RTO must be approved in advance (time off must be made up)

Travel/Territory

• Must live within 30 minutes of territory
• Travelers must secure nearby housing
• Weekly in-office visit required
• Mileage reimbursed
