Licensed Social Worker (LICSW or LCSW)
Passionate about working with the elderly? Looking for a schedule that fits your life? Were seeking a licensed Social Worker to join a collaborative care team dedicated to supporting older adults in the community.
About the Role:- Focus exclusively on the geriatric population
- Conduct psychosocial assessments and create individualized care plans
- Participate in daily interdisciplinary team meetings
- Coordinate facility admissions and discharges as needed
- Educate and support families with community resources
- Minimum of 1 year of experience in Social Work providing traditional clinical or case management services with a geriatric population
- Active LICSW or LCSW license required
- Strong communication and teamwork skills
- Compassionate approach to supporting older adults and their families
- Monday-Friday, 8 AM4 PM schedule
- No nights, weekends, or holidays
- Meaningful work improving the quality of life for seniors
