Community Health Worker – Clinic

Overall Responsibility

A community health worker provides support to a caseload of up to 30 persons with chronic health conditions. The work is done primarily in the community with specific chronic health populations who need additional support to improve their health status. The worker works closely with medical providers, primary care, behavioral health and dental teams to improve patient care and outcomes.

Key Tasks and Responsibilities
  • Use PRAPARE tool to assess Social Determinants of Health (SDOH) issues that are potentially impacting healthcare status.
  • Engage persons in a productive working relationship to address SDOH and provide needed support.
  • Develop a treatment plan and document progress toward the goals.
  • Review progress with the treatment plan quarterly and make course corrections as needed with the treatment team.
  • Engage natural supports in the treatment of persons served in the program.
  • Assist clients in effectively using available resources both in the center and the community by assisting with scheduling appointments, tracking the status of referral completion.
  • Provide timely, accurate and quality documentation.
  • Manage time well by being on time for appointments, attending staff team meetings, and completing documentation on time.
  • Work to improve the overall health status of individuals served.
  • Able to prioritize workload.
  • Work as part of an integrated team.
  • Work with the Quality Improvement Team and provide support for quality improvement PDSA projects.
  • Must be able to sit and drive for a significant part of a work day.

  • Possesses a basic knowledge of the health delivery system to include the Missouri Department of Mental Health, Federally Qualified Health Centers, Substance use disorders treatment facilities and community resources.
  • Has a working knowledge of chronic health conditions, serious mental illness and developmental disabilities.
  • Maintains a professional appearance and dresses appropriately as the situation dictates.
Physical Requirements

Travel primarily in a 30 mile radius of the office. Extensive documentation in an electronic health record. Lifting requirements are approximately 15 pounds and the ability to provide physical support to some clients on a non-routine basis.

Education and Licensure

Meets at least one of the following:

• Meets the criteria to bill as a community support.

• Has a bachelor's degree in social work, psychology, or closely related fields or is a licensed nurse.

• Equivalent experience may be substituted on the basis of one year of experience for each year of required education.

Class E Driver’s License and a good driving record.

Certifications:

BLS, Nonviolent Crisis Intervention, Level 1 Medication Aid.

Experience Requirements

At least one year in case management with adults and/or youth with serious and persistent mental illness or substance use disorders.
