OverviewInfu Care Rx is seeking an experienced Bilingual Care Transition Coordinator & Territory Manager to join our team in Miami, FL region. The successful candidate must be bilingual, reside in Miami, FL, and be willing to commute.
Roles & Responsibilities- Develop and execute regional sales strategies to achieve growth goals and company objectives.
- Build and maintain strong relationships with physicians, discharge planners, case managers, and key referral sources.
- Promote Infu Care Rx’s specialty infusion programs, including parenteral nutrition, anti‑infective therapies, and chronic disease management.
- Educate healthcare professionals on Infu Care Rx’s infusion therapy programs, patient‑centered care model, and value‑based services.
- Identify and develop new referral opportunities within the assigned territory.
- Strategically plan field visits to optimize coverage, reduce travel time, and enhance productivity.
- Represent Infu Care Rx at community and professional events (e.g., health fairs, educational programs, and in‑services) to increase brand awareness.
- Collaborate with internal pharmacy, nursing, and administrative teams to ensure timely and high‑quality patient care.
- Provide market insights, trends, and competitive updates to leadership to support strategic planning.
- Coordinate referrals for home infusion services, ensuring accurate documentation and smooth transitions from hospital to home.
- Review medical records, verify insurance benefits, and assess patient eligibility for home infusion prior to discharge.
- Communicate with physicians, discharge planners, and case managers to design individualized care transition plans.
- Educate patients and families about infusion therapy, insurance coverage, and financial responsibilities.
- Identify and promptly report risk management or quality issues to leadership.
- Ensure compliance with HIPAA, state, federal, and accreditation standards.
- Stay informed about payer policies, reimbursement guidelines, and home infusion industry trends.
- Bachelor’s degree or minimum of 3 years of relevant industry experience (specialty pharmacy, infusion, or healthcare sales).
- Must be bilingual in English and Spanish.
- Nursing or clinical background is a plus but not required.
- Strong understanding of infusion therapy, specialty pharmacy, or healthcare transitions.
- Excellent communication, presentation, and relationship‑building skills.
- Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
- Ability to work independently, manage multiple priorities, and meet deadlines.
- Willingness to travel throughout the assigned territory as needed.
