The Chaplain acts as a community liaison for patients linking them to local parish/congregation/ synagogue, while providing spiritual counseling in cooperation with the Interdisciplinary Team's plan of care.

Essential Functions

Assess spiritual factors related to patient care and intervenes appropriately.

Establishes of a spiritual care plan for patient / families.

Provide accurate and appropriate documentation and submit daily.

Coordinates spiritual services within structure of team.

Insure documentation includes patient's religious preference and assistance offered.

Provides spiritual counseling to meet needs in accordance with the patient’s and family’s acceptance of this service, and in a manner consistent with patient and family beliefs and desires.

Administers sacraments (or arranges for them to be administered) as requested and performs funeral or memorial services upon request by the family.

Provide spiritual support of patient/family on individual or group basis.

Makes visits as necessary according to assessed needs, within established time frames.

Provides explanation of spiritual aspects of care to interdisciplinary team members.

Acts as liaison with professional religious community by making reasonable efforts to arrange for visits of clergy and members of religious organizations in the community to patients who request such visits.

High comfort level in dealing with terminal illness.

Demonstrates knowledge of hospice levels of care, Medicare and Medicaid Hospice Benefits by providing appropriate documentation when the need arises.

Develops and assists with educational services in the community.

Develops, supervises and evaluates the annual Hospice Memorial Services.

Exhibit respect for individual religious preference/practice.

Exhibit high comfort level in dealing with terminal illness.

Maintaining the ability to extend hospitality and patience while assisting patients, families, physicians and peers under stressful circumstances.

Other Duties

Participates as an active member of the interdisciplinary team identifying and addressing spiritual matters through case consultation and case finding.

Support services must be organized to meet the needs of patients, families, and others that may have vital interest in the patient's well-being.

The services must conform to the plan of care.

Patient/family records should indicate the extent to which support services are used.

The plan of care should be consistent with the patient/family needs.

Provides Team Manager with weekly schedule of patient appointments and follow-up documentation.

Educates religious community to the collaboration of Hospice and clergy.

Ensure effective communication with Interdisciplinary Teams.

Provide orientation on a regular basis to new employees.

Consistently able to work calmly and maintain effectiveness in situations of high stress.

Participate with on-call rotations.

Ability to travel to any location throughout South Florida as needed and expand territory as business needs dictate.

Maintain your required licenses, certifications and mandatory skill updates.

Comply with all policies, local, state and federal laws and regulations.

Provide other duties of healthcare team member.

Perform other duties as assigned.

Supervisory Responsibility

May serve as an interim department leader depending on need

Physical Requirements

Must be able to lift and/or move up to 50 pounds and push/pull up to 250+ pounds, walk, climb stair or ladders, stand on feet for extended periods of time, etc.

Knowledge & Experience Requirements

Master’s Degree or Doctorate Degree in Divinity, Theology or related field required.

Four (4) units of CPE (Clinical Pastoral Education) or equivalency are preferred.

Recognition or confirmation by your church, congregation, or synagogue as such and in good standing.
1-2 years of experience in related area.

Hospice experienced preferred.

Must have knowledge of computer office/clinical software.

Must be able to read, write and understand the English language.
