Position Summary: The person in this position will be responsible to act as the primary consumer advocate for the program assigned. The person in this position keeps abreast of progress, and coordinates all services delivered to the consumer and family internally and through external referral.
Essential Job Functions- Administer evidence based assessments as need
- Conduct Needs Assessment
- Develop Service Plans in collaboration with the consumer and family
- Conduct monthly Service Plan reviews
- Document all clinical services and activities in accord with licensing, Medicaid, and Joint Commission standards and agency policy and procedure
- Conduct individual case management sessions
- Provide vocational education and skills building to the consumer as needed
- Perform activities on behalf of the consumer and family, including attendance at external appointments (e.g., court appearances, school staffing, etc.), as needed
- Provide referral and linkage for the consumer and family, as needed
- Prepare monthly Progress Reports, as required, for assigned consumers for courts, probation officers, caseworkers, etc.
- Coordinate consumer and family care with the treatment team and with external providers
- Responsible for the development of the discharge plan in coordination with other treatment team members
- Arrange transportation for consumers, as necessary
- Provide case management to additional populations, as needed
- Participate in case conference
- Understands HIPAA, Professional Ethics, Consumer Rights and Confidentiality
- Completes all orientation and mandated training activities, including E-Learning Courses, within required time frames, and in accordance with the criteria established for this position.
- Participate in performance improvement activities

- Perform any other duties as assigned.
- Certification as a certified case manager in good standing and recognized by the Florida Certification Board is preferred at time of hire;
Preference will be given to those candidates who are currently Certified;
- Graduation from an accredited bachelors program with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is required
- Two years of previously supervised work experience in providing individual/family therapy is preferred
- Bilingual is preferred.
- Must have reliable means of transportation
- Must have familiarity with Miami-Dade County.
- Maintains confidential files and records.
- The ability to identify and evaluate an individual’s strengths, barriers, problems, and needs for the development of a Wellness and Recovery Plan
- The ability to evaluate the impact of substance use and mental health problems on the consumer’s functioning in major life areas
- Knowledge of the inter-relationship among substance use, mental health, and primary care disorders.
- The ability to conceptualize cases within a developmental framework for the purpose of treatment planning, intervention, and discharge planning
- Knowledge in the understanding of programs built on proven methods that ensure effective treatment
- Demonstrate proficiency is the administration and interpretation of assessments, as needed
- The ability to coordinate and integrate multiple services, as well as interface and communicate with internal and external providers
- The ability to provide information to individuals and families concerning alcohol or other drug use, mental health problems, and the available services and resources
- Ability to use trauma informed care principles including ensuring consumer safety, preventing/avoiding re-traumatization, maximizing trustworthiness, enhancing consumer choice, maximizing collaboration, and emphasizing empowerment
