We are seeking a Speech‑Language Pathologist to provide in‑home care for Medicare Part B patients. In this role, you will assess and treat speech, language, cognitive‑communication, and swallowing disorders, creating individualized therapy plans to help patients improve communication, swallowing, and overall quality of life.

• Assess patients in their homes and create individualized therapy plans.
• Provide skilled interventions for speech, language, cognition, and swallowing.
• Educate patients and caregivers on strategies to improve communication and daily functioning.
• Document progress and ensure compliance with Medicare Part B regulations.
• Collaborate with the home health/ therapy care team to coordinate patient care.
• Adjust treatments based on patient progress and needs.

• Active Speech‑Language Pathology (SLP) license in the state of practice.
• Certificate of Clinical Competence (CCC‑SLP) required.
• Experience in home health or outpatient therapy preferred.
• Knowledge of Medicare Part B documentation and regulations.
• Strong assessment, treatment planning, and patient education skills.
• Excellent communication and interpersonal abilities.
• Reliable transportation and a valid driver's license.
• Comfortable working independently and managing a flexible, home‑based schedule.
• Proficiency with electronic medical records (EMR) and basic computer skills.
• CPR and other relevant certifications as required.
