Summary The Outreach Navigator will act as a Linkage to Care Specialist playing a vital role in coordinating initial appointments and ensuring seamless integration of HIV/HCV‑positive patients into medical care. This position requires direct patient contact, involving counseling, appointment scheduling, and linkage coordination. The specialist collaborates with hospital teams, the Department of Health (DOH), and community resources to enhance healthcare access and equity.

Responsibilities

  • Coordinate with providers and community groups to schedule Jackson Insurance Navigator at community events, health fairs, and other gatherings.
  • Assist Project Director with scheduling assignments for outreach and in‑reach initiatives.
  • Conduct outreach to consumers with a health insurance and coverage option focus.
  • Distribute fair and impartial information concerning enrollment in QHPs, the availability of premium tax credits and cost‑sharing reductions in accordance with federal tax laws and enrollment in public programs.
  • Educate the public about the value of health insurance for the individual and family in culturally and linguistically appropriate ways.
  • Inform consumers, particularly the uninsured and underinsured, about health insurance and coverage options available through healthcare.gov.
  • Aid consumers in understanding health insurance terms, such as premiums, deductibles and co‑insurance.
  • Provide materials and explanations about the ten required Essential Health Benefits of all Affordable Care Act policies.
  • Inform consumers about the application process, required documentation needed, mandated requirements and any exemption criteria.
  • Explain program eligibility rules for advance premium tax credits, cost sharing reductions, Medicaid or CHIP.
  • Describe the various methods available to purchase and enroll in QHP.
  • Address questions regarding access to enrollment methods and the submission of enrollment documentation.
  • Explain the enrollment criteria for purchasing insurance through healthcare.gov.
  • Administer tools to assess insurance needs for individuals, including all persons who reside in a household, to identify possible eligibility for insurance options.
  • Assist individuals with the enrollment process.
  • Provide referrals to other social service providers or Navigators as needed.
  • Handle post‑enrollment inquiries issues and grievances.
  • Maintain expertise in Navigator duties and program specifications.
  • Gather information for verification, statistical and grant reporting purposes.
  • Monitor consumer presence at locations and activities to ensure that outreach efforts in targeted neighborhoods remain viable.
  • Demonstrate behaviors of service excellence and CARE values (Compassion, Accountability, Respect and Expertise).
  • Perform all other related job duties as assigned.

Experience

  • Generally requires 3 to 5 years of related experience.

Preferred Qualifications

  • Prior experience in HIV/AIDS or HCV services, healthcare coordination, or patient advocacy is advantageous.
  • Strong interpersonal, communication, and problem‑solving skills to engage effectively with diverse populations.
  • Proficiency in data entry and electronic medical records.
  • Sensitivity to cultural and socioeconomic diversity.
  • Willingness to undergo DOH training and certification in HIV/AIDS testing and counseling.
  • Bilingual Spanish and/or Creole.

Education

  • High school diploma is required.
  • Bachelor’s degree in Public Health Social Work, or Healthcare Administration preferred.

Credentials Valid license or certification is required as needed, based on the job or specialty.
