Case Worker 1 (H) – University of Miami
Full‑time Case Worker 1 role within the UHealth Department of Pediatrics – Mailman Center for Child Development in Miami.
Responsibilities- Conduct patient evaluations to develop effective treatment plans.
- Ensure patients enroll in insurance and free or low‑cost social service programs.
- Learn Affordable Care Act enrollment process to assist patients with care transition.
- Identify and resolve real or perceived barriers to care.
- Assist with patient‑provider communication and medication adherence education.
- Conduct secondary prevention and lifestyle management interventions.
- Provide clients with information on available clinical trial opportunities and facilitate linkages to research staff.
- Educate and counsel patients around life situation issues and find additional information from websites and agencies.
- Maintain knowledge of community health and welfare resources.
- Maintain confidentiality of all records, data, and information as required by HIPAA regulations.
- Document filing and actions taken according to departmental guidelines and standards.
- Assist with record audits for documentation and record compliance.
- Adhere to University policies and procedures and safeguard University assets.
- Travel to homes, daycares and other natural environment locations as necessary.
- Serve as single point of contact for families in need of early intervention services.
- Contribute to development of a family support plan and monitor implementation.
- Assist parents of eligible children in gaining access to early intervention services.
- Provide education to families on services available in the community.
- Collaborate with personnel in community‑based agencies to develop appropriate intervention strategies.
- Maintain a resource and referral file to assist families in finding timely programs.
- Maximize continuity of care provided to enhance developmental potential.
- Coordinate activities with other case‑related medical and non‑medical personnel.
- Facilitate authorization of appropriate level of services for each child and family enrolled in the program.
- Document services provided in the prescribed manner and complete paperwork prescribed by the Department.
- Completes required paperwork as needed for input into data and billing systems.
- Bilingual in English and Spanish preferred.
- Bachelor’s Degree in psychology, social work, health education, interdisciplinary sociology, early childhood, child development or special education.
- One year of relevant experience with young children and families in a teaching, case management or counseling role, or with individuals with special needs and/or developmental delays.
- Skill in completing assignments accurately and with attention to detail.
- Ability to analyze, organize and prioritize work under pressure while meeting deadlines.
- Ability to process and handle confidential information with discretion.
- Ability to work evenings, nights, and weekends as necessary.
