- Conduct patient evaluations to obtain information necessary for the development of an effective treatment plan.
- Ensure patients have access to insurance and social service programs for which they are eligible.
- Provide patient reminders of upcoming appointments and act as a liaison between patients and providers.
- Identify and resolve real or perceived barriers to care.
- Educate and counsel patients on issues related to their health and life situations.
- Maintain current knowledge of community health, welfare resources, and any other programs available to patients.
- Handle patient communications and contact healthcare facilities for medical records.
- Address care gaps with the nurse navigator.
- Prepare, track, and process medical records and pathology specimens; review and file records for completeness using tracking systems for accurate documentation.
- Provide administrative support to the department by answering phones, addressing inquiries, directing calls or taking messages, copying, faxing, emailing, and mailing records while ensuring confidentiality.
- Adhere to University and unit‑level policies and procedures and safeguard University assets.

Core Qualifications- Education:
High School diploma or equivalent required. - Experience:
Minimum 1 year of relevant experience required. - Certification and Licensing:
