Responsibilities
  • As a private duty nurse on our home health team, care for medically complex patients, primarily children, at home.
  • Provide personalized nursing care for pediatric and/or adult patients with complex needs.
  • Coordinate care plans with physicians, therapists, and other team members.
  • Monitor patient progress and update care plans as needed.
  • Educate and support families with in-home care techniques.
  • Document visits and care notes in our EMR (electronic charting system).
  • Collaborate with your clinical supervisor and participate in case conferences.
  • Be a trusted part of a long-term care team focused on safety, comfort, and continuity.
Qualifications
  • Active RN license
  • CPR certification
  • Clinical experience preferred—but new grads are welcome
  • Excellent communication and critical thinking skills
  • A love for patient-centered care, especially in a home environment
  • Confidence, compassion, and a calm presence during routine care and emergencies

RN

Duties (Summary)

A Registered Nurse (RN) administers skilled nursing care to patients on an intermittent basis in their place of residence, in accordance with physician orders and the plan of care per state Practice Act. In this role, you will report to the Director of Nursing (DON).

  • Develops, delivers/implements, revises, and evaluates individualized patient-centered care plans under physician supervision.
  • Provides case management to an assigned group of patients and ancillary staff.
  • Understands and submits required data collection sets and other documentation within required time frames.
  • Makes the initial evaluation visit and reevaluates nursing needs per state and payor requirements.
  • Acts as an advocate for patient welfare and coordinates care between patients, families, agency and other providers.
  • Performs services requiring substantial specialized nursing skill per competency level.
  • Demonstrates problem solving and interpersonal communication skills.
  • Identifies patient care preferences and ensures interventions meet goals of treatment.
  • Initiates preventive and rehabilitative nursing procedures; prepares clinical notes; attends case conferences.
  • Informs physician and staff of changes in condition; counsels patient and family on nursing needs and home health strategies.
  • Observes and reports symptoms and reactions; performs medication reconciliation.
  • Participates in in-service programs and may supervise other nursing personnel.
  • Administers medications per policy and orders; provides written instructions for aides and other staff.
  • Refers cases to therapists and Medical Social Worker when needed; participates in clinical record reviews and QAPI as assigned.
