Nurse Navigator, Primary Care

Overview

Nurse Navigator, Primary Care, FT, 8:30A-5P at Baptist Health. The Nurse Navigator acts as liaison between patients, physicians and the care team, coordinating care across the continuum and providing education and information to patients, support for decision making, advocacy, and efforts to improve throughput and patient satisfaction. The Nurse Navigator monitors patient outcomes and participates in quality improvement activities within a multidisciplinary healthcare setting.

Estimated salary range for this position is $79,331.20 - $ per year, depending on experience.

• Coordinate care across the continuum and facilitate decision making for patients and families.
• Educate and provide information to patients and significant others; advocate for patients’ needs.
• Support care teams to improve patient throughput and satisfaction.
• Monitor patient outcomes and participate in quality improvement activities.
• Collaborate with a multidisciplinary health care team to positively impact patient outcomes and experiences.

• Degrees:
  • Bachelor of Science in Nursing – BSN.

• Licenses &
  
  Certifications:
  • Neonatal Resuscitation Program (NRP).
  • Pediatric Advanced Life Support (PALS).
  • Basic Life Support (BLS).
  • Registered Nurse (RN).
  • Advanced Cardiovascular Life Support (ACLS).

• Additional Qualifications:
  • Active Registered Nurse license.
  • Three years of clinical experience in a hospital, managed care environment, or medical office.
  • Proficient in computer applications and typing skills.
  • Experience with Electronic Medical Record systems preferred.
  • Bilingual preferred.
  • Preferred certification in area of expertise.
  • For South Entities, RNs hired after 2/2012 with an Associate degree have 5 years to complete the BSN degree.
  • Life Support Certificate (BLS, NRP, PALS, ACLS) as per department standard.
