Nurse Navigator 2, BHMG Cardiology, FT, 8:30A-5P Overview
The Nurse Navigator (NN) acts as a multidisciplinary team leader and liaison between patients, family, physicians, clinical care teams, community organizations and BHSF leadership. The NN is responsible for leading coordination of care across the continuum, providing education, support and resources to patient/family, facilitate shared decision making, patient advocacy, ensuring timely access to quality multidisciplinary care and patient satisfaction. The NN will monitor individuals, populations and program outcomes and engage in quality improvement activities.
Estimated salary range: $86,465.60 - $ per year, depending on experience.
- Lead coordination of care across the continuum and serve as a multidisciplinary team leader and liaison among patients, families, physicians, clinical care teams, community organizations and leadership.
- Provide education, support and resources to patients and families; facilitate shared decision making and patient advocacy; ensure timely access to quality multidisciplinary care and patient satisfaction.
- Monitor individuals, populations and program outcomes; engage in quality improvement activities.
- BSN (Bachelor of Science in Nursing) required.
- Active Registered Nurse (RN) license.
- Certifications:
Advanced Cardiovascular Life Support (ACLS), Basic Life Support (BLS), Neonatal Resuscitation Program (NRP), Pediatric Advanced Life Support (PALS). - Five years nursing experience in key relevant areas strongly preferred:
Med/Surg, Oncology Care (Inpatient/Outpatient Oncology, Infusion, Radiation, BMT, etc.), Patient Navigation or related service (Case Management, Care Coordination, Social Work, Population Health), or specific Disease Sites (Breast, GI, GU, Neuro, etc.). - Experience working in a hospital, managed care or medical office setting preferred.
- Proficiency with Microsoft Office, EMR applications (Word, Excel, SharePoint, TEAMS, HEDIS, STAR) and typing.
- Bilingual English/Spanish preferred.
- Life Support Certificate per department standard (BLS, NRP, PALS, ACLS).
- Minimum required experience: 5 years.
