Position: Case Manager Registered Nurse - Full Time

Case Manager Registered Nurse - Full Time

Responsibilities
  • Identify patient risk factors or obstacles to care and discharge/readmission risk.
  • Evaluate the plan of care regularly by chart review and patient interviews, and collaborate with the medical team to facilitate the patient’s movement through the system.
  • Educate patients and families on the progression of care.
  • Serve as a liaison between patients, families, and healthcare personnel to ensure necessary care is provided promptly, effectively, and fiscally responsibly.
  • Promote quality care to ensure patients receive medically appropriate services in appropriate status and stay standards.
  • Facilitate ongoing insurance authorization for continued stay and regulatory notifications and patient signatures per policy.
  • Maintain knowledge regarding insurance reimbursement policies.
  • Plan and facilitate discharge and transition plans that meet the physical, social, and emotional needs of the patient.
  • Adhere to University and unit-level policies and procedures and safeguard University assets.
Minimum Qualifications Education
  • Graduate from an accredited school of nursing, Bachelor’s degree (BSN).
Certification and Licensing
  • Valid Florida Registered Nurse License and BLS certification from the American Heart Association required.
Experience
  • Minimum 2 years of relevant experience, including at least one year in Hospital Case Management or nursing.
  • Working knowledge of patient assessment and medical terminology.
Knowledge, Skills and Attitudes
  • Effective oral and written communication skills.

    Ability to recognize, analyze, and solve a variety of problems.
  • Ability to analyze, organize and prioritize work under pressure while meeting deadlines.
  • Ability to maintain effective interpersonal relationships.
