• Performs on‑site screening clinical assessments to determine appropriate patient treatment plans for residents at client facilities by the completion of a comprehensive assessment
• Functions independently to perform age‑appropriate history and physical for complex acute, critical, and chronically ill perioperative patients
• Orders and interprets diagnostic and therapeutic tests relative to patient’s age‑specific needs
• Assists with the provision of care in accordance with facility, state, and federal regulations
• Monitors the effectiveness of interventions
• Maintains relationships with all appropriate contacts in facilities
• Educates family members, clients, and prospective accounts regarding services and programs
• Assists with the development and maintenance of physician relations, as assigned
• Completes weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner
• Other duties as assigned by Medical Director

• Current Nurse Practitioner License
• Degree from an accredited Nurse Practitioner Program
• Advanced Cardiovascular Life Support (ACLS)
• Current infection control certification
• Strong clinical collaboration with physicians
• Proficient computer skills
• PACS and EMR experience preferred
• Committed to professional growth and learning
• Clean driving record
• Personally owned vehicle required
• Local travel required
• Not listed on the OIG exclusion list
