Licensed Practical Nurse (LPN) - Full Time

Job Summary:

Unity Health and Rehabilitation Center is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic team. Responsibilities:

• Provide skilled nursing care to residents, including administering medications, wound care, and vital sign monitoring

• Collaborate with the interdisciplinary team to develop and implement individualized care plans

• Perform physical assessments, obtain vital signs, and document resident conditions

• Participate in patient education and discharge planning

• Communicate effectively with residents, families, and healthcare team members

• Maintain accurate and up-to-date records and reports

Requirements:

• Current LPN license in the state of Florida

• Minimum of 1 year of experience in a long-term care or rehabilitation setting

• Ability to work effectively in a fast-paced environment with minimal supervision

• Strong communication and interpersonal skills

• Compassionate and patient-centered approach to care

• Ability to pass a drug test
