Position: Full Case Manager Specialist

A minimum of High School Diploma or its equivalent is required. Associate Degree or Bachelors preferred.

  • Familiarity with the Full Case Management System as well as with the Child Welfare Dependency System in Miami Dade County, preferred.
  • Familiarity with Child Welfare and Child Welfare related computer programs, preferred.

Hourly Rate Range:

$14.00/hour to $16.50/hour based on diploma/degree.

100% coverage of employee only Medical, Dental, Vision, STD, LTD and Life Insurance.

Skills/Abilities
  • Must be computer literate and able to use the systems including but not limited to Word, Excel, FSFN, CJIS and ASK system.
  • Must have good listening, communication and writing skills.
  • Must be willing to learn.
  • Must be willing and able to work well with others and cooperate in the sharing of resources and information in order to obtain the final work product.
  • Must be open minded and willing to follow instructions.
  • Must be able to lift 50 lbs.
  • Must be able to sit, stoop, run, climb, and actively participate in activities related to the age of the group.
  • Must have a good driving record for at least 5 years.
  • Must be willing and able to work well with others and cooperate in all aspects of the Case Management Program.
Specific Areas of Responsibility to Position
  • Assist Full Case Management with referrals sent to the Clinical Department.
  • Follows up with the Clinical department to ensure children and/or parents have been linked with services.
  • Assist Full Case Management with tracking and scheduling medical, dental and medical management appointments.
  • Follows up with providers to obtain treatment reports.
  • Provides secretarial support to Unit (emails, scheduling appointments, copies, obtaining reports, printing reports and information from FSFN and ASK, attain school, medical and educational records etc.).
  • Helps unit members send documents to QA/QI point person for filing in system.
  • Assist Full Case Management with monitoring and supervising children in the office who are waiting for placement, appointments or for other reasons.
  • Assist Full Case Management with conducting supervised visitation.
  • Maintains a valid driver’s license and notifies his/her Supervisor of any situation causing his/her license to be inactive.
  • The Full Case Manager Specialist is expected to have a reliable vehicle and have appropriate auto insurance.
  • Forwards any and all documentation provided to the Full Case Manager Specialist by the Child(ren)’s providers to the Case Manager upon receipt of those documents or within 24 hours of receipt of those documents.
  • Completes all assignments as requested by the Full Case Manager Supervisor and/or Administrators in a timely manner.
  • Assist Full Case Management by attending court with clients, when applicable.
  • Assist Full Case Management with creating transition plans for IL, if applicable.
  • Assist Full Case Management by attending staffings, as needed.
  • Assist Full Case Management with OTI/ICPC and home study packets.
  • Assist with removals and accompanying Full Case Manager on out of county assignments.
  • Assist Full Case Management with permanency staffings.
  • Assist Full Case Management with transporting clients, as a secondary function of the position.
  • Assist Full Case Management with facilitating communication between the Child(ren)’s providers and the agency.
  • Participates in the agency’s Quality Assurance/Quality Improvement Program.
  • Contributes to the understanding and acceptance of cultural diversity within the agency and the community.
  • Complies with all relevant JCAHO standards and regulations.
  • Utilizes the agency’s resources in a fiscally conservative manner.
  • Performs other duties as assigned by Supervisor.
