• Deliver resident-centered nursing care aligned with individualized care plans
• Administer medications and treatments safely and accurately
• Monitor resident health and communicate changes to RNs and physicians
• Support residents with respiratory or rehabilitative needs as part of their care journey
• Collaborate with a multidisciplinary team to ensure holistic, high-quality outcomes
• Provide leadership, support, and mentorship to CNAs to maintain care standards
• Foster a safe, clean, and respectful environment for residents and staff

• Active Licensed Practical Nurse (LPN) license in Ohio (required)
• CPR certification required
• Experience in skilled nursing, rehab, or long-term care preferred
• Strong clinical judgment, attention to detail, and adaptability
• Compassionate, dependable, and team-oriented personality
• Excellent communication and interpersonal skills
