Licensed Clinical Social Worker / LCSW at Rehab Without Walls® Neuro Rehabilitation

• Are you an experienced LCSW passionate about providing counseling services that support an individual’s physical, mental, cognitive, and emotional adjustment through recovery?
• You provide therapeutic interventions and resource recommendations that address patient’s goals to achieve their personal, career, and independent living goals in the most integrated setting possible
• You value professional autonomy, teamwork, communication, and collaboration to maximize patient’s recovery

• Guide and counsel individuals and families in crisis, ranging from the unemployed to children who need foster homes to elderly people who have no one to care for them
• Assist clients in obtaining government funds, education, or treatment
• Secure information such as medical, psychological, and social factors contributing to client’s situation, evaluate these and client capacities, and ask clients to explain problems to determine nature and degree of problem
• Help patients and families through individual or group conferences regarding plans for meeting needs, and aid clients to mobilize inner capacities and environmental resources to improve social functioning
• Help clients modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and their role in creating them
• Develop service plans designed to restore patients to optimum social and health adjustment within their capacity
• Refer clients to community resources and other organizations, liaise with other social service agencies, and utilize community resources to assist patients in resuming life in community or learning to live within limits of disability
• Prepare patient histories, service plans, and reports; review plans and perform follow‑up to determine quantity and quality of service provided and status of the client’s case
• Maintain client records in compliance with organizational goals and objectives, as well as external regulatory guidelines
• Other duties as assigned

• Master’s Degree in Social Work
• Five or more years of clinical social work experience
• Valid and current Licensed Clinical Social Worker or Licensed Medical Social Worker license
• Ability to make psycho‑social assessments and develop and implement viable care plans
