. Father Murray, A Villa Center is seeking a Social Services Designee/PRSC to join their team! The Social Services Designee/PRSC provides person‑centered social, emotional, and psychological support to skilled nursing residents and promotes continued growth and development to each individual’s full potential.

• Keep informed about new and existing community resources.
• Communicate daily with staff regarding admissions and discharges to various units, facilities, hospitals, community, and home settings.
• Demonstrate knowledge of regulations / standards and discuss advanced directives, financial powers of attorney, guardianship, and surrogate decision‑making with residents or their representatives.
• Document discharge planning as an ongoing review and initiate ongoing communication with the resident and family to assess discharge needs.
• Ensure home‑care needs are met by the time of discharge, aiming for arrangements completed 24 hours prior to discharge when the date is known.
• Complete all required assessments and documentation for each resident in a timely manner.
• Perform all other duties as assigned.

• Ability to read, write, and communicate in English.
• Strong verbal and written communication skills for performing required tasks.

• Must have a high school diploma or equivalent.
• Experience working with geriatric residents is preferred.
• Working knowledge of Medicare, Medicaid, HMO, and private insurance coverage criteria.
• Knowledge of accrediting organization standards, IDPH regulations, and state and federal regulations related to skilled care facilities.

• Must be able to move intermittently throughout the workday.
• Must be able to push, pull, or lift a minimum of 25 pounds and carry such weight over a minimum distance of 50 feet, with a height of at least 5 feet.
