Full-Time Post-Acute - Lake Michigan Region with Great Benefits We are hiring a dedicated, full-time Advanced Practice Clinician (NP/PA) with a strong background in Internal or Adult Medicine to join our team near Custer/Ludington, Michigan. This is a rewarding opportunity to deliver high-quality, safe, and cost-effective care within our local post-acute care communities. Role & Responsibilities:
Provide comprehensive care, including H&Ps, admissions, progress evaluations, transfers, and discharges, for patients in post-acute settings. Collaborate with an interdisciplinary team to ensure a seamless transition of short-term rehab patients to the most appropriate level of care (home, assisted living, memory care, etc.). Work closely with hospice, palliative care, and home care agencies to ensure secure patient transitions. Maintain a high standard of quality and safety in patient care with an action-oriented "can do" attitude.
