What You'll Do

• Round regularly and manage wound care for patients at assigned skilled nursing facilities (SNFs).
• Collaborate with wound care nursing staff to assess and treat residents with wounds.
• Deliver high-quality, patient-centered, comprehensive wound care services.
• Complete timely and accurate documentation for all patient encounters.
• Be available for phone and secure messaging consultations with facility nursing staff.
• Document care in Theoria's Chart Easy™ EHR and interface with facility EHRs (Point Click Care , Matrix Care, American Health Tech).
