LVN​/LPN – LTC​/SNF

Job Summary:

Provides direct nursing care to residents in a Long-Term Care/SNF setting under supervision, ensuring compliance with established protocols and policies. Focuses on delivering quality patient-centered care while adhering to organizational standards.

Responsibilities:

• Assist staff nurses in executing treatment and care plans for residents.
• Maintain accurate documentation of patient care activities and observations.
• Follow facility policies, including mandatory attendance and punctuality standards.
• Support patient and family-centered care to enhance the overall experience.
• Adhere to infection control and safety procedures, including proper use of PPE.
• Report significant patient changes to supervising nurses promptly.

Qualifications:

• Minimum 2 years of experience in Long-Term Care or Skilled Nursing Facilities.
• Current BLS certification from AHA.
• Valid Michigan LVN/LPN license.
• Ability to work a 12-hour shift and meet scheduling requirements.
• Demonstrated understanding of patient and family-centered care principles.
• Fingerprinting clearance for Michigan LTC preferred.
