LVN​/LPN – LTC​/SNF

Job Summary:

Provide nursing care and support to patients in a long-term care setting, ensuring compliance with facility policies and industry standards. Assist staff nurses in delivering quality patient-centered care by adhering to established protocols and safety procedures.

Responsibilities:

• Support staff nurses in providing comprehensive patient care in accordance with hospital policies and procedures.
• Recognize and demonstrate an understanding of patient and family-centered care principles.
• Follow strict attendance policies, including timely shift start and adherence to absence protocols.
• Document patient information accurately and promptly, ensuring compliance with regulatory requirements.
• Assist with patient treatments, vital signs, and routine nursing tasks as directed by supervising staff.
• Maintain professional appearance by adhering to the facility’s dress code, including matching scrubs.

Qualifications:

• Verify a minimum of 2 years of experience in long-term care or skilled nursing facilities.
• Hold a valid State license as an LVN or LPN in Michigan.
• Possess current BLS certification from the American Heart Association.
• Demonstrate knowledge of LTC/Skilled Nursing facility protocols and patient care standards.
• Be able to provide documentation of MI LTC fingerprinting (preferred).
• Commit to adhering to the institution’s attendance and safety policies.
