Wound Nurse RN​/LPN

As a Wound Care Nurse (RN/LPN), you’ll play an important role in protecting residents’ skin integrity, preventing pressure injuries, and supporting healing through individualized care and clinical expertise.

• Assess, monitor, and document wounds to evaluate healing progress, identify changes, and determine appropriate interventions

• Develop, implement, and update individualized wound care plans based on resident needs and provider orders

• Perform wound treatments, dressing changes, and other prescribed interventions in accordance with clinical standards

• Identify residents at risk for pressure injuries and skin breakdown and implement prevention strategies

• Monitor wound healing and recognize signs of infection or complications, promptly communicating concerns to the appropriate healthcare providers

• Collaborate with the Director of Nursing, providers, nursing team members, and interdisciplinary team to coordinate wound care and improve resident outcomes

• Educate nursing team members on wound prevention, treatment techniques, skin assessments, and documentation standards

• Ensure accurate and timely wound assessments, treatment records, and clinical documentation

• Monitor the effectiveness of wound care interventions and recommend changes when needed

• Support compliance with state and federal regulations, infection prevention standards, and clinical best practices

• Administer medications and provide other nursing treatments as needed

• Supervise and support nursing assistants in providing safe, compassionate resident care

• Current Registered Nurse (RN)/ Licensed Practical Nurse (LPN) license in good standing

• Experience in wound care, skilled nursing, or long-term care preferred

• Wound care certification preferred

• Strong knowledge of wound assessment, treatment, pressure injury prevention, and skin integrity management

• Ability to recognize changes in resident condition and respond appropriately

• Knowledge of infection prevention, clinical documentation, and applicable state and federal regulations

• Ability to develop and implement individualized care plans and evaluate treatment outcomes

• Strong communication, organization, and clinical judgment

• Ability to educate and support nursing team members in delivering quality care

• Medication administration and IV certification as required for the position
