** Job Summary
** The SSCA supports individuals receiving B3 and waiver services by coordinating care, linking resources, and monitoring service delivery. The role focuses on ensuring access to necessary medical, social, and community-based services to support independence, inclusion, and overall well-being. Additionally, provides care management services to adults diagnosed with serious mental illness and intellectual and/or developmental disabilities.

** Key Responsibilities
*** Assist with assessments and person-centered service plans
* Coordinate and link individuals to internal and external services
* Monitor service delivery and ensure plans are implemented as written
* Document services, progress notes, and required reports
* Conduct periodic follow-up and reassessments
* Advocate for individuals and support access to resources
* Collaborate with families, providers, and community partners
* Support Direct Support Staff and interdisciplinary teams
* Maintain compliance with MDHHS, Medicaid, and agency standards
* Participate in required training and team activities-###
** Additional Responsibilities
*** Maintain professionalism in all interactions
* Support onboarding and training of new staff as needed
* Travel within the service area and statewide as required
* Perform other duties as assigned-###
** Qualifications
* ** High school diploma or associate degree
* 1 year of experience working with individuals with serious mental illness and/or intellectual and/or developmental disabilities.
* Strong organization, communication, and coordination skills-###

* Frequent standing, walking, and computer use
* Ability to lift up to 25 pounds
