Registered Nurse, Long-Term Care (LTC) travel assignment in Middlebury, VT.

• Setting: 98-bed long-term care facility with post-acute skilled nursing rehab unit
• Floating required; schedule varies; must be able to work all holidays; no guaranteed weekends unless specified in contract

• Administer medications and treatments per orders; monitor patient status and respond to changes in condition
• Perform phlebotomy, EKGs, IV starts and IV pump management as needed to support post-acute care
• Provide direct nursing care for post-acute skilled nursing rehab patients; assist with transfers and transports within the facility
• Document care in the electronic medical record; communicate with the unit leader/DO/N and interdisciplinary team
• Collaborate with LNA/CNA/PCT staff to support care plans and deliver dementia-capable, safe patient care

• License on file at time of submission
• Basic Life Support (BLS) certification
• Minimum 1 year of clinical experience; 1+ year in post-acute/skilled nursing rehab preferred
• Prior travel experience required
• EPIC EMR experience within the past 2 years
• Dementia-capable care certification preferred
