As a Housing Case Manager, you will provide individualized support to people experiencing or at risk of homelessness by:
- Completing shelter intakes and comprehensive assessments.
- Developing housing stability plans with each participant.
- Connecting guests with housing, healthcare, mental health, substance use treatment, employment, education, childcare, legal services, and public benefits.
- Assisting with housing searches, rental applications, landlord communication, lease education, and move-in coordination.
- Advocating for clients by attending appointments and collaborating with community partners.
- Maintaining accurate case notes and documentation.
- Managing a caseload of approximately 12–18 individuals, meeting with each participant weekly or more frequently as needed.
- Participating in Coordinated Entry, housing coalition meetings, staff meetings, and ongoing professional development.
- Supporting shelter operations and participating in an on-call rotation every 3–4 weeks.
We’re seeking someone who is:
- Compassionate and committed to serving people experiencing homelessness.
- Skilled at building trusting relationships with individuals from diverse backgrounds.
- Experienced in case management, social services, peer support, crisis intervention, or a related field.
- Familiar with trauma-informed, person-centered care.
- Organized, dependable, and comfortable using electronic documentation systems.
- Able to work independently while collaborating effectively with a team.
