The Epilepsy Foundation of Connecticut (EFCT) is seeking a passionate and dedicated Client and Community Outreach Coordinator to provide direct support to individuals and families affected by epilepsy while building strong relationships with healthcare providers, community partners, and volunteers across the state.

Key ResponsibilitiesClient Support and Care Coordination
  • Provide education, emotional support, and resource referrals to individuals and families managing an epilepsy diagnosis.
  • Offer care coordination services that address social determinants of health, as needed.
  • Maintain a regular on-site presence at partnering Epilepsy Centers at local hospitals (minimum twice weekly).
  • Accurately document all client interactions using human service software.
Support Group Network Oversight
  • Recruit and train volunteer support group leaders.
  • Monitor and support the effectiveness of groups to ensure participants are receiving appropriate peer support.
Community Engagement and Education
  • Cultivate relationships with diverse community providers and promote EFCT programs.
  • Represent EFCT at health fairs, conferences, and community events.
  • Facilitate in-person and virtual seizure recognition and first aid trainings.
Qualifications and Skills

Required:

  • Bachelor’s degree in a human services or public health field or certification as a CT Community Health Worker or 4–6 years’ experience in a relevant field.
  • Valid driver’s license, automobile insurance, and reliable transportation.
  • At least 2 years of experience providing direct support services.
  • Excellent verbal and written communication skills.
  • Demonstrated cultural competence and experience working with individuals with disabilities and diverse communities.
  • Ability to represent EFCT professionally and ethically.
  • Strong organizational skills; able to manage multiple projects and meet deadlines.
  • Proficiency with Microsoft Office; familiarity with human service databases is a plus.
  • Ability to maintain confidentiality and work both independently and collaboratively.
  • Willingness to travel statewide and occasionally work evenings or weekends.
Compensation & Benefits
  • Annual Salary : $55,000
  • Retirement : 3% employer match through SIMPLE IRA
