Key ResponsibilitiesClient Support and Care Coordination- Provide education, emotional support, and resource referrals to individuals and families managing an epilepsy diagnosis.
- Offer care coordination services that address social determinants of health, as needed.
- Maintain a regular on-site presence at partnering Epilepsy Centers at local hospitals (minimum twice weekly).
- Accurately document all client interactions using human service software.
- Recruit and train volunteer support group leaders.
- Monitor and support the effectiveness of groups to ensure participants are receiving appropriate peer support.
- Cultivate relationships with diverse community providers and promote EFCT programs.
- Represent EFCT at health fairs, conferences, and community events.
- Facilitate in-person and virtual seizure recognition and first aid trainings.
Required:
- Bachelor’s degree in a human services or public health field or certification as a CT Community Health Worker or 4–6 years’ experience in a relevant field.
- Valid driver’s license, automobile insurance, and reliable transportation.
- At least 2 years of experience providing direct support services.
- Excellent verbal and written communication skills.
- Demonstrated cultural competence and experience working with individuals with disabilities and diverse communities.
- Ability to represent EFCT professionally and ethically.
- Strong organizational skills; able to manage multiple projects and meet deadlines.
- Proficiency with Microsoft Office; familiarity with human service databases is a plus.
- Ability to maintain confidentiality and work both independently and collaboratively.
- Willingness to travel statewide and occasionally work evenings or weekends.
